MANAGEMENT OF PATIENT HEALTH INFORMATION

ReCreation Medical Centre is committed to maintaining the confidentiality of your health information.  We comply with the Australian Privacy Principles. For more details, please ask for a copy of our privacy brochure or to see our brochure.

This general practice collects information from you for the primary purpose of providing quality health care. We require you to provide us with your personal details and a full medical history so that we may properly assess, diagnose and treat illnesses and medical conditions, ensuring we are proactive in your health care. To enable ongoing care, and in keeping with the Privacy Act 1988 and Australian Privacy Principles, we wish to provide you with sufficient information on how your personal information may be used or disclosed and record your consent or restrictions to this consent.

 

Your personal information will only be used for the purposes for which it was collected or as otherwise permitted by law, and we respect your right to determine how your information is used or disclosed.

 

The information we collect may be collected by a number of different methods and examples may include: medical test results, notes from consultations, Medicare details, data collected from observations and conversations with you, and details obtained from other health care providers (e.g. specialist correspondence).

FEES

ReCreation Medical Centre is a private billing practice and payment is required on the day of service.

 

Due to increasing costs of running the practice and recent issues with dramatic inflation, the clinic has made the decision to increase fees to be more in line with the Consumer Price Index for 2022.

 

We are also phasing out the use of cash payments. If patients wish to pay with cash, please have the exact change on the day, or a split payment of cash and card can be made.

Please note that under the current Medicare MBS rules you are only eligible for a rebate if you have been to the clinic face to face in the last 12 months.

Current 2023 Consult Fees (Dr Mark Choong):

 

Standard Medicare Item 23 (<20 minutes) – $102.00 

*15 minute appointment slot for standard consult

 

Long Medicare Item 36 (<40 minutes) – $180.00

 

Extended Medicare Item 44 (>40 mins) – $250.00 to $360.00

Vaccines:

We stock a range of Travel and other vaccines which may be an additional fee.  Prices are available from reception.

APPOINTMENTS

Appointments may be made by phoning us on 9509 5922 or clicking here. Please make a separate appointment for each person requiring a consultation, regardless if you are seeing the same doctor at the same time.

 

Our standard appointment time is 15 minutes.  If you have a number of issues to discuss with the doctor or require a longer appointment, please advise reception at the time of booking.

 

If you are unable to make an appointment, please call 9509 5922 so that it can be offered to another patient.

AFTER HOURS AND EMERGENCY CARE

For emergencies, always dial 000.

 

Recreation Medical Centre provides limited access to a doctor after hours care.  Please phone 9509 5922 and you will be diverted to the mobile of our doctor on call.

TELEPHONE CALLS

It may not be possible for your doctor to take a call at the time you phone, please let us know if the matter is urgent and your call will be transferred to the Practice Nurse.

 

For non-urgent matters, reception staff will take your details and pass a message onto your doctor for them to respond at the end of their appointment session.

COMMUNICATION SERVICES

Patients who require communication assistance are asked to let the reception staff know when making the appointment.  We will then arrange an interpreter through the appropriate service.

TEST RESULTS

Please make a follow up appointment within 2-3 days to discuss results with your doctor.  We will send an SMS alert for any result marked ‘Discuss’ by the doctor. 

 

To ensure patient confidentiality results will not be provided over the phone.

REPEAT PRESCRIPTIONS AND REFERRALS

An appointment is required to obtain repeats for prescriptions and referrals.

REMINDER SYSTEM

ReCreation Medical Centre is committed to preventative healthcare.  You have the option of registering to receive healthcare reminders that are appropriate to your care.  Please advise staff if you do not wish to be contacted.

PARKING

Limited parking is available directly outside the clinic on High Street.  Longer term parking is available in the council car park at the rear of the building. 

ZERO TOLERANCE

All our staff are trained and dedicated to serve you and you will be treated with courtesy and respect at all times. In return, we ask that you and anyone that you bring with you to the Practice treat our administrative and clinical staff with the same courtesy and respect.

 

We have a ZERO TOLERANCE approach to any verbal, aggressive and violent abuse and behaviour towards our staff or other patients. We respectfully advise you that abuse and/or violence will not be tolerated.

 

If a patient or anyone a patient brings with them to the Practice, is abusive, aggressive or violent towards our staff and/or other patients the police will be called and may result in you being removed from our Practice list.
